Refresh Channel and Doc Names & Emojis

When one changes the name of a channel and/or its associated emoji, that name and emoji doesn't automatically update wherever it's been previously referenced in another post with a #. This can be very cumbersome to go back and change all prior posts with the reference, and of course confusing to users if we don't. Can we change this to auto refresh or repopulate whenever a channel name is changed?
